Our HIPAA privacy notice

Federal law gives you rights over your medical information. This notice explains how My IV Doctors and its clinicians may use it and how you can see, correct and control it.

Please read this notice carefully

It describes how medical information about you may be used and shared, and how you can get access to it. It applies to My IV Doctors and to the independent, licensed clinicians who treat patients through our platform.

What counts as protected health information

Protected health information (PHI) is any information that identifies you and concerns your physical or mental health, the care you receive or payment for that care, whether in the past, now or in the future. The law requires us to keep it private, to give you this notice of our duties and practices and to follow the version of the notice that is currently in effect.

Uses that do not need your written permission

Your care
Sharing information with the clinicians, pharmacies and labs treating you, so your care is planned and coordinated. For example, a clinician sends your prescription to a pharmacy.
Billing
Charging and collecting for services, including processing payments through our payment partners.
Running the service
Quality checks, clinician supervision, training and day-to-day administration of the platform.
Legal requirements
Disclosures that federal, state or local law requires, such as public health reporting or reports of suspected abuse or neglect.
Serious threats
Disclosures needed to prevent or reduce a serious, imminent threat to your health or safety, or the public’s.

Uses that need your written permission

We must have your written authorization before most uses of psychotherapy notes, before using your information for marketing and before any sale of your PHI. You can cancel an authorization in writing whenever you like; the cancellation does not undo anything we already did while relying on it.

Your rights

See and get a copy
You can look at, and ask for a copy of, the health information used to make decisions about your care.
Ask for a correction
If you think something in your record is wrong or missing, you can ask us to change it.
Get a list of disclosures
You can ask for a list of certain times we have shared your PHI.
Ask us to limit what we share
You can ask us to restrict a use or disclosure. We will consider it but may not always be able to agree.
Choose how we contact you
You can ask us to reach you a particular way, such as by a certain phone number or at another address.
Get this notice on paper
You can ask for a printed copy at any time, even if you agreed to receive it electronically.

What we promise

We keep your PHI private and secure, follow the practices in this notice and tell you promptly if a breach may have exposed your information. Beyond the uses described here, we will only use or share your information if you give us written permission.

If this notice changes

We may revise this notice, and a revised notice can cover information we already hold as well as information we receive later. The current version, with its effective date, is always on this page.

How to raise a concern

If you believe your privacy rights were violated, email us at team@myivdoctors.com or file a complaint with the Office for Civil Rights at the U.S. Department of Health and Human Services. Filing a complaint will never be held against you.
