Rybelsus is semaglutide in tablet form, made by Novo Nordisk and first approved by the FDA in 2019. It lowers blood sugar by boosting insulin release when glucose is high, reducing glucagon and slowing digestion. Its FDA label covers two uses in adults with type 2 diabetes: improving blood sugar control alongside diet and exercise, and reducing the risk of heart attack, stroke and cardiovascular death in those at high cardiovascular risk.
The Rybelsus label now also includes Ozempic tablets, a newer formulation of oral semaglutide offered in the U.S. since 2026 in 1.5, 4 and 9 mg strengths. If you're starting oral semaglutide for the first time, your clinician may prescribe the Ozempic tablet instead. If you're already stable on Rybelsus, you may be able to continue it or switch to the matching Ozempic tablet dose.
Rybelsus is approved only for adults with type 2 diabetes. It is not approved for weight loss, and it isn't prescribed for weight loss alone, though some people with diabetes do lose weight while taking it. If weight is your main concern, your clinician can talk through options that are approved for weight management.

Rybelsus is started low and increased monthly. The first strength helps you tolerate the medicine; it isn't expected to control blood sugar.
Set the tablet and a small glass of plain water by your bed. On waking, swallow one whole tablet with no more than 4 ounces of water. Don't split, crush or chew it.
Then give it at least 30 minutes before breakfast, coffee, juice or any other pill. Many people use that half hour to shower, walk the dog or check glucose. If your schedule changes, for example on a weekend or a trip, keep the same order of events even if the clock time shifts.
Because Rybelsus is taken on an empty stomach with a strict wait afterward, it interacts with your morning routine as much as with other drugs. Bring a full medicine list to your visit.

There is no need to switch if Rybelsus is working and available to you. If you do switch, the label pairs Rybelsus 7 mg with Ozempic 4 mg and Rybelsus 14 mg with Ozempic 9 mg. Never switch during the first 30 days of treatment.
The 3 mg starter dose is there to help your stomach adjust. The label notes it isn't effective for glucose control, so improvement usually begins after moving to 7 mg.
It's meant to be taken in the morning on an empty stomach, before any food or drink other than a small sip of water. Taking it after meals is likely to reduce absorption.

A1C reflects roughly the past three months of blood sugar, so it's usually rechecked a few months after you reach an effective dose. Home glucose readings often start to improve sooner, once you move past the 3 mg starter dose.
Semaglutide slows stomach emptying, which matters during anesthesia. Tell your surgeon and anesthesia team you take it, and follow their instructions about pausing.
Let your clinician know and keep up with regular eye exams. A rapid drop in blood sugar can temporarily worsen retinopathy, so vision changes should be reported promptly.
