Wegovy HD is a single-dose pen containing 7.2 mg of semaglutide, three times the usual 2.4 mg Wegovy maintenance dose. The FDA approved it in March 2026 for long-term weight reduction in adults, used with a reduced-calorie diet and more physical activity.
The label is specific about who can move up: adults who have taken 2.4 mg weekly for at least four weeks, have tolerated it and for whom more weight reduction is clinically indicated. It is approved only for weight reduction, so it does not replace the 2.4 mg dose for heart-risk reduction or the liver disease indication.
For most people, the question arrives months into treatment, when weight loss has slowed on 2.4 mg and side effects have settled. Your clinician weighs your progress, how well you tolerate the current dose and your health goals before suggesting the higher dose.
Wegovy injections climb in four-week steps: 0.25 mg, 0.5 mg, 1 mg and 1.7 mg, then the 2.4 mg maintenance dose. Wegovy HD is an optional extra rung above that. The label calls 7.2 mg the maximum weekly dose.
A bigger dose is not automatically better. The 7.2 mg dose was studied for 72 weeks in adults with obesity before it was approved. Some people see added benefit from it, but the chance of side effects also rises, so some find the extra effect worthwhile while others prefer to stay at 2.4 mg.
