Zepbound is the brand name Eli Lilly uses for tirzepatide when it is prescribed to manage weight. It carries two FDA indications. The first is to reduce excess body weight and keep it off long term in adults with obesity, or adults with overweight who also have at least one weight-related health condition. The second is to treat moderate to severe obstructive sleep apnea (OSA) in adults with obesity. Both uses are paired with a reduced-calorie diet and more physical activity.
Because it is a brand product, every dose is made by the manufacturer under FDA oversight. It comes in several presentations: a single-dose autoinjector pen with a hidden needle, single-dose vials and four-dose KwikPens and vials. The medicine inside is the same. What differs is how you measure and inject it.

In two 52-week trials described in the FDA label, adults with obesity and moderate to severe OSA took 10 mg or 15 mg of Zepbound or a placebo. The apnea-hypopnea index, a count of breathing interruptions per hour of sleep, fell by about 25 to 29 events per hour on Zepbound, compared with about 5 on placebo. Between 42% and 50% of people on Zepbound reached remission or mild OSA without symptoms.
For OSA, the label lists 10 mg or 15 mg weekly as the maintenance dose. Zepbound does not replace a sleep study, and you should not stop CPAP or another prescribed therapy unless the doctor managing your sleep apnea tells you to.
All Zepbound presentations deliver the same strengths: 2.5, 5, 7.5, 10, 12.5 and 15 mg. Your choice mostly comes down to how you like to inject and how often you want a refill.
Everyone begins at 2.5 mg once a week for four weeks, which is a starter dose only. The dose then goes to 5 mg and can rise by 2.5 mg after at least four weeks at each level, up to 15 mg. For weight management, maintenance doses are 5, 10 or 15 mg. If a higher dose upsets your stomach too much, your clinician can keep you at a lower one.
In the main weight trial in adults without diabetes, the 15 mg dose led to average weight loss of 20.9% at 72 weeks versus 3.1% on placebo. In adults with type 2 diabetes, the average was 14.7% versus 3.2%. Your own result may differ.
Digestive side effects are the most common and usually ease as your body adjusts. The label also lists less common but serious risks your clinician screens for.
